This privacy policy summarizes the policies and practices of Weis Insurance Agency, LLC regarding the collection, disclosure, and confidentiality of your nonpublic personal information.  At Weis Insurance Agency, LLC, we are committed to protecting your privacy.  We want you to understand what information we collect about you and how we use that information.  This privacy policy applies to our relationship with you if you are an individual who obtains products or services from us for your personal, family, or household purposes.

Information We Disclose to Affiliates and Nonaffiliated Third Parties

Weis Insurance Agency may exchange such information only with other insurance related parties that are similarly obligated under state and federal privacy laws and have in place the appropriate procedures to keep all treatments and exchanges of your information within the requirements of these laws.

We may disclose the following kinds of nonpublic personal information about you:

·                      Information we receive from you on applications or other forms, such as your name, address, social security number, assets, income, and beneficiary information;

·                      Information about your transactions with us, our affiliates or others, such as your policy coverage, premiums, and payment history; and

·                      Information we receive from a consumer reporting agency, such as your creditworthiness and credit history

We may disclose nonpublic personal information about you to the following types of third parties:

·                      Financial service providers, such as life insurers, automobile insurers, mortgage bankers, securities broker-dealers, and insurance agents

We may also disclose nonpublic personal information about you to non-affiliated third parties as permitted by law.

As we place your insurance with these carriers, both our agency and the carriers work together (as well as individually) to retain uses for only those activities required to underwrite, issue, and service your policy, as well as conduct claims activities – should that be necessary on your behalf.  We restrict access to nonpublic personal information about you to those employees who need to know that information to provide products or services to you.  We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your nonpublic personal information.

Confidentiality of Health Information

It is often necessary to obtain personal health information in order to underwrite insurance and process claims for various types of personal insurance coverage.  We recognize your concerns about the security of that information and want to assure you that any personal health data you provide to us will be held in strict confidence.  We will not disclose nor share your personal health information for marketing or any other unauthorized purpose.  We may disclose or share your personal health information for underwriting or administrating your policy or claim or for other purposes permitted by law.

If you prefer that we do not disclose nonpublic personal information about you to non-affiliated third parties, you may opt out of these disclosures, that is, you may direct us not to make those disclosures (other than disclosures permitted by law).  If you wish to opt out of disclosures to non-affiliated third parties, please inform us at the time of application or call us at 217-342-9777 or 1-800-337-8909.

If a party to a joint policy account or other relationship with us directs us not to share non-experience information within our corporate family, we will treat that direction as applying to all parties to the joint policy, account or other relationship.

We will also adhere to any State privacy law requirements not addressed herein.
